This is a form for opting out of data sharing via an IT system called SystmOne that some GPs use. It is to do with local data sharing for your own direct medical care.

care.data is nothing to do with your direct medical care, it is for ‘secondary uses’. To opt out of care.data go to ‘How to opt out‘.
